Dementia developed in a renal transplant recipient who was receiving prednisone (15 mg per day) and azathioprine (50 mg per day). Cerebral computed axial tomography showed multiple areas of infarction. Cerebral angiography demonstrated bilateral vessel changes suggestive of diffuse vasculitis. A search for other causes of vasculitis revealed none. This case demonstrates that isolated angiitis of the brain may develop and progress despite medication capable of suppressing rejection of a transplanted kidney.
