Mental illness has been a significant public health challenge in Thailand, where there is a shortage of specialists. Consequently, community nurses (CNs) who are not specialists have been tasked with enhancing mental health outcomes in the primary care system of the country. This study aims to shed light on CNs' lived experiences of caring for psychiatric patients within the primary care units (PCUs).
A phenomenological research approach and in-depth interviews were conducted with nine CNs responsible for psychiatric patient care purposefully selected at nine PCUs in southern Thailand, including Nakhon Si Thammarat, Krabi, Phangnga, and Chumporn provinces, between March and December 2019. Data analysis was carried out using Colaizzi's method.
The experiences of CNs were expressed through three main themes: "Confronting role expansion in long-term psychiatric care responsibilities," "Insecurity in role ambiguity," and "Duty of supportive caring for marginalized people."
Thai CNs face significant stress when providing care for psychiatric patients, grappling with their acknowledged lack of specialization and essential expertise in the field. Alongside the issue, they knew that supportive caring for marginalized people is their duty to humanity. Primary care systems must recognize CNs' vital role in enhancing psychiatric patient care. Comprehensive training empowers them, fostering confidence and enabling better connections and care management. This can lead to improved patient outcomes and overall care quality.
精神疾病一直是泰国面临的重大公共卫生挑战,该国精神科专家短缺。因此,非专科的社区护士(CNs)被委以重任,以提高该国初级保健系统的精神卫生服务成效。本研究旨在深入了解 CNs 在初级保健单位(PCUs)中照顾精神科患者的亲身经历。

CNs 的经验主要通过三个主题表达出来:“长期精神科护理责任下的角色扩展”、“角色模糊带来的不安全感”和“为边缘化人群提供支持性护理的责任”。
泰国的 CNs 在为精神科患者提供护理时面临着巨大的压力,他们认识到自己在该领域缺乏专业知识和必要的专业技能。除此之外,他们知道为边缘化人群提供支持性护理是他们的人道主义责任。初级保健系统必须认识到 CNs 在加强精神科患者护理方面的重要作用。全面的培训赋予他们权力,增强他们的信心,并促进更好的联系和护理管理。这可以改善患者的预后和整体护理质量。
