: Total knee arthroplasty (TKA) improves balance performance in patients with osteoarthritis; however, balance deficit and fall incidence after TKA have been reported. This study aimed to determine the effects of combined balance exercises on knee range of motion (ROM), balance, gait, and functional outcomes during the acute phase after TKA. : A total of 42 participants were randomly assigned to either the combined balance group ( = 21) or the general physical therapy (control) group ( = 21). The combined balance exercise group performed exercise programs for 30 min per session, five times a week for 4 weeks (20 sessions), and the control group completed general physical therapy, which included active simple exercise. Measurements were performed before and after the 4 weeks of training to assess changes in the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), knee joint ROM, static balance, timed up-and-go (TUG), and 10 m walk test (10 MWT). : The combined balance exercise group demonstrated significant improvements post intervention ( < 0.05) for all outcomes. The time × group interaction effect for the WOMAC scores showed statistically significant interaction effects for pain, stiffness, and physical function; the static and dynamic balance values showed statistically significant interaction effects for CEA, PL, AV, and TUG; and gait ability showed a significant interaction effect for the 10 m walk test ( < 0.05). : This study confirmed that combined balance training with general physical therapy has a positive effect on ROM, static and dynamic balance, gait, and functional outcomes in the acute phase post TKA and that combined balance exercise can be proposed as a rapid rehabilitation intervention with general physical therapy following TKA.
全膝关节置换术(TKA)可改善骨关节炎患者的平衡表现;然而,TKA 后仍存在平衡缺陷和跌倒发生率。本研究旨在确定联合平衡练习对 TKA 后急性期膝关节活动度(ROM)、平衡、步态和功能结果的影响。
共有 42 名参与者被随机分配到联合平衡组(n=21)或常规物理治疗(对照组)(n=21)。联合平衡运动组每周进行 5 次、每次 30 分钟的运动方案,共进行 4 周(20 次),对照组则完成常规物理治疗,包括主动简单运动。在 4 周的训练前后进行测量,以评估 Western Ontario 和 McMaster 大学骨关节炎指数(WOMAC)、膝关节 ROM、静态平衡、计时起立行走测试(TUG)和 10 米步行测试(10 MWT)的变化。
联合平衡运动组在干预后(<0.05)所有结果均有显著改善。WOMAC 评分的时间×组交互效应显示疼痛、僵硬和身体功能具有统计学显著的交互效应;静态和动态平衡值的 CEA、PL、AV 和 TUG 具有统计学显著的交互效应;步态能力在 10 米步行测试中显示出显著的交互效应(<0.05)。
本研究证实,与常规物理治疗相结合的联合平衡训练对 TKA 后急性期的 ROM、静态和动态平衡、步态和功能结果有积极影响,并且联合平衡运动可以作为 TKA 后与常规物理治疗相结合的快速康复干预措施。
