Abstract

The present study evaluated the clinical outcomes and satisfaction of patients undergoing fresh homologous osteochondral transplantation in the knee as a salvage method.  We analyzed eight knees from seven male patients who underwent fresh homologous osteochondral transplantation by a single surgeon. Their follow-up period ranged from 10 months to 5 years and 5 months. Clinical outcomes included the scores on the International Knee Documentation Committee (IKDC) and on the quality-of-life item of the Knee and Osteoarthritis Outcome Score (KOOS-QoL).  The sample consisted of complex cases since all operated knees had undergone previous surgeries. Functional improvement was variable, with six out of the seven operated patients showing statistically significant clinical improvement according to the IKDC score, and a single patient reported being moderately satisfied with the procedure. The quality-of-life item from the KOOS score improved in all patients. There was no failure, need for reintervention, or infection.  Fresh homologous osteochondral transplantation is a safe salvage method in our setting to treat large lesions and those with failed previous procedures. Despite the small sample of this case series, most clinical outcomes were positive and had no complications.

本研究评估了膝关节新鲜同种异体骨软骨移植作为挽救方法的临床疗效及患者满意度。我们分析了7名男性患者的8个膝关节,这些患者均由同一位外科医生进行了新鲜同种异体骨软骨移植。他们的随访期为10个月至5年零5个月。临床疗效包括国际膝关节文献委员会(IKDC)评分以及膝关节和骨关节炎转归评分(KOOS-QoL)中的生活质量项目评分。该样本由复杂病例组成,因为所有接受手术的膝关节此前均接受过手术。功能改善情况各不相同,7名接受手术的患者中有6名根据IKDC评分显示出统计学上显著的临床改善,1名患者报告对手术中度满意。所有患者KOOS评分中的生活质量项目均有所改善。未出现失败、再次干预需求或感染情况。在我们的研究环境中,新鲜同种异体骨软骨移植是治疗大的病变以及先前手术失败的病变的一种安全的挽救方法。尽管该病例系列样本量较小,但大多数临床疗效是积极的且无并发症。
