Primary splenic diffuse large B-cell lymphoma (PS-DLBCL) is a rare manifestation of malignant lymphoma. Although DLBCL is the most common subtype of non-Hodgkin lymphoma, primary splenic involvement is uncommon. Additionally, a gastrosplenic fistula at initial presentation is even more rare and poses a diagnostic challenge for the radiologist. The clinical and imaging picture can be identical to a splenic abscess with a secondary gastrosplenic fistula formation (GSF), leading to a diagnostic conundrum. Hence, radiologists need to be familiar with this entity for correct diagnosis and appropriate clinical management.
原发性脾脏弥漫性大B细胞淋巴瘤（PS-DLBCL）是恶性淋巴瘤的一种罕见表现形式。虽然弥漫性大B细胞淋巴瘤是非霍奇金淋巴瘤最常见的亚型，但原发性脾脏受累并不常见。此外，初诊时出现胃脾瘘更为罕见，给放射科医生带来了诊断挑战。其临床和影像学表现可能与伴有继发性胃脾瘘形成（GSF）的脾脓肿相同，导致诊断难题。因此，放射科医生需要熟悉这一实体，以便做出正确诊断和进行适当的临床管理。
