Abstract

OBJECTIVE

To analyze the diagnostic value of a combined test of anti-cyclic citrullinated peptide antibody (CCP), anti-keratin antibody (AKA), anti-carbamylated protein antibody (Carp antibody), and rheumatoid factor (RF) in the early diagnosis of rheumatoid arthritis (RA).

METHODS

Sixty cases of RA admitted to our hospital from 2021 to 2022 (observation group) were selected, along with 50 cases of healthy physical examiners (control group). The results of CCP antibody, AKA antibody, Carp antibody, and rheumatoid factor in both groups were analyzed: the concentration of CCP antibody, AKA antibody, Carp antibody, and rheumatoid factor in each group at different levels; the comparison of various testing methods with the "gold standard" test; and the ROC curve analysis of CCP antibody, AKA antibody, Carp antibody, and rheumatoid factor in each group.

RESULTS

The concentrations of CCP antibody, AKA antibody, Carp antibody, and rheumatoid factor were significantly higher in the observation group than in the control group (p < 0.05). Furthermore, there was a significant increase in the concentrations of CCP antibody, AKA antibody, Carp antibody, and rheumatoid factor from the mild to moderate group (p < 0.05), as well as from the moderate to severe group (p < 0.05). Additionally, when comparing the mild and severe groups, there was a significant elevation in the concentrations of CCP antibody, AKA antibody, Carp antibody (p < 0.05), and rheumatoid factor (p < 0.05).

CONCLUSION

Anti-cyclic citrullinated peptide antibody, AKA antibody, Carp antibody, and rheumatoid factor combined tests have high sensitivity and specificity in the diagnosis of RA.

目的

分析抗环瓜氨酸肽抗体(CCP)、抗角蛋白抗体(AKA)、抗瓜氨酸化蛋白抗体(Carp抗体)及类风湿因子(RF)联合检测在类风湿关节炎(RA)早期诊断中的价值。

方法

选取2021年至2022年我院收治的60例RA患者(观察组),以及50例健康体检者(对照组)。分析两组CCP抗体、AKA抗体、Carp抗体及类风湿因子的检测结果:不同水平下每组CCP抗体、AKA抗体、Carp抗体及类风湿因子的浓度;各种检测方法与“金标准”检测的比较;每组CCP抗体、AKA抗体、Carp抗体及类风湿因子的ROC曲线分析。

结果

观察组CCP抗体、AKA抗体、Carp抗体及类风湿因子的浓度显著高于对照组(p<0.05)。此外,从轻度到中度组,以及从中度到重度组,CCP抗体、AKA抗体、Carp抗体及类风湿因子的浓度均显著升高(p<0.05)。另外,比较轻度和重度组时,CCP抗体、AKA抗体、Carp抗体(p<0.05)及类风湿因子(p<0.05)的浓度均显著升高。

结论

抗环瓜氨酸肽抗体、AKA抗体、Carp抗体及类风湿因子联合检测在RA诊断中具有较高的敏感性和特异性。
