Pure chondral fragments in adolescents are very rare, especially, fragments from the femoral trochlea. While satisfactory short-term outcomes after fixation were reported in several reports, long-term outcomes have still been unknown. Our objective is to evaluate the long-term outcomes after fixation for pure chondral fragment from the trochlea.
MATERIALS & METHODS: Three adolescent patients (aged 13, 14, and 14 years) with pure chondral fragment from the trochlea underwent internal fixation using bio-absorbable pins at our institution. All patients were followed up for more than eight years. At final follow-up, all three patients were assessed by radiographs and magnetic resonance imaging (MRI) as well as clinical scores, including Lysholm questionnaires, and IKDC (International Knee Documentation Committee) score, KOOS (Knee injury and Osteoarthritis Outcome Score).
At final follow-up (12, 8, and 9 years postoperatively) all three patients were able to continue to play sports without any restriction. Radiographs showed no osteoarthritic changes and MRI showed good integration with the surrounding native cartilage and smooth surface in all cases. In all cases, Lysholm score and IKDC score were over 95, and most of KOOS were over 90.
Fixation for pure chondral fragment from the femoral trochlea resulted in successful healing in adolescent patients with excellent radiographic and clinical outcomes in the long-term.
青少年单纯软骨碎片非常罕见,尤其是来自股骨滑车的碎片。虽然有几份报告报道了固定术后令人满意的短期结果,但长期结果仍然未知。我们的目的是评估股骨滑车单纯软骨碎片固定术后的长期结果。
3例青少年患者(年龄分别为13岁、14岁和14岁),其股骨滑车有单纯软骨碎片,在我们机构接受了使用生物可吸收针的内固定治疗。所有患者均随访超过8年。在末次随访时,通过X线片、磁共振成像(MRI)以及临床评分对所有3例患者进行评估,临床评分包括Lysholm问卷、国际膝关节文献委员会(IKDC)评分、膝关节损伤和骨关节炎疗效评分(KOOS)。
在末次随访时(术后12年、8年和9年),所有3例患者都能够继续不受限制地进行运动。X线片显示无骨关节炎改变,MRI显示所有病例中碎片与周围天然软骨良好整合且表面光滑。所有病例中,Lysholm评分和IKDC评分均超过95分,大多数KOOS评分超过90分。
股骨滑车单纯软骨碎片固定术使青少年患者获得了成功愈合,长期的影像学和临床结果均良好。
