Transcatheter edge to edge repair (TEER) has been proven to be safe and effective for mitral regurgitation (MR) caused by acute myocardial infarction (AMI). However, for some specific conditions, such as papillary muscle rupture complicating AMI, the feasibility of TEER is not yet fully determined. A 74-year-old woman with coronary disease developed acute MR and heart failure after AMI. Transesophageal echocardiography confirmed severe MR due to papillary muscle rupture. The patient underwent emergency TEER, which successfully reduced MR and improved symptoms. This case highlights the life-saving potential of the Dragonfly TEER system in treating acute MR after MI, particularly in patients with extremely high surgical risk.
