This study aims to assess the effect of using medical waste rubber bung (MWRB) for pin-tract management in patients with open tibial fractures treated with external fixators (EFs).
A retrospective analysis of 91 patients with open tibial fractures admitted to our hospital over a three-year period was conducted to compare and statistically characterize overall PTI incidence, PTI rate across five different pin-tract locations, time (days) to the first occurrence of infection, and Checketts-Otterburn classification.
Among the enrolled 91 patients, 88 met the criteria. After excluding deaths and losses to follow-up, they were divided into two study groups, with no significant difference in overall PTI incidence. Group A exhibited a significantly lower rate of severe infection and prolonged time to initial infection compared to Group B (both < 0.05). Group A also had a significantly lower rate of PTI at the tibial telangiectasia than Group B.
The study underscores that compression in EF management is necessary to significantly reduce the incidence of severe PTIs, especially in the tibial metaphysis, and to delay the onset of initial infection among patients with open tibial fractures.
本研究旨在评估使用医用废弃橡胶塞(MWRB)对接受外固定架(EFs)治疗的开放性胫骨骨折患者进行针道管理的效果。
对我院三年内收治的91例开放性胫骨骨折患者进行回顾性分析,比较并统计总体针道感染(PTI)发生率、五个不同针道位置的PTI发生率、首次发生感染的时间(天)以及Checketts - Otterburn分类。
在纳入的91例患者中,88例符合标准。排除死亡和失访病例后,将他们分为两个研究组,总体PTI发生率无显著差异。与B组相比,A组的严重感染率显著较低,初始感染时间延长(均P < 0.05)。A组在胫骨毛细血管扩张处的PTI发生率也显著低于B组。
该研究强调,在外固定架管理中进行加压对于显著降低严重PTI的发生率是必要的,尤其是在胫骨近端干骺端,并可延迟开放性胫骨骨折患者初始感染的发生。
