To evaluate the microarchitecture changes in the trabecular bone around (mesial and distal) the tooth after root canal treatment on panoramic radiographs by measuring the fractal dimension (FD) value change with fractal analysis (FA).
One hundred two patients (69 females, 33 males) with a mean age of 33.12 (18-69) years were included. These patients had a clinically healthy, radiologically successful root canal-treated mandibular premolar/molar tooth treated at least 1 year ago. A healthy, untreated contralateral tooth with the same tooth number had to be present (control tooth). Patients were divided into four groups according to their ages (18-24, 25-34, 35-44, and 45 +), teeth into three groups according to treatment time (1-3 years, 4-6 years, and 7-8 years), and three groups according to tooth number (35-45, 36-46, 37-47). FD measurements were made at two rectangular Region of Interest (ROI) with 20 × 60 pixels selected on the mesial and distal of the root with the Image J version 1.3 software program on panoramic radiographs. The comparison of the FD values of the treated and control teeth was performed via the Wilcoxon signed ranks test.
The FD values of mesial and distal regions of root canal-treated molars were statistically significantly higher than the control teeth (p < 0.05). The treatment time, tooth number, and sex and age of the patients did not have a statistically significant effect on the increase in FD value (p > 0.05). There was a correlation between the mesial FD value changing and treatment year (r = - 0.21, p = 0.035).
After the first year of root canal treatment, quantitatively measurable structural changes were observed in the trabecular bone in both the mesial and distal regions of the tooth in all age groups and sexes in the posterior teeth of the mandible.

纳入102例患者(69例女性,33例男性),平均年龄33.12(18 - 69)岁。这些患者有一颗临床健康、放射学检查显示根管治疗成功的下颌前磨牙/磨牙,且治疗时间至少在1年前。必须存在一颗同侧未治疗的健康同名牙(对照牙)。患者按年龄分为四组(18 - 24岁、25 - 34岁、35 - 44岁和45岁及以上),牙齿按治疗时间分为三组(1 - 3年、4 - 6年和7 - 8年),按牙位分为三组(35 - 45、36 - 46、37 - 47)。使用Image J 1.3软件程序在全景X线片上,在牙根近中和远中选择两个20×60像素的矩形感兴趣区域(ROI)进行FD测量。通过Wilcoxon符号秩检验对治疗牙和对照牙的FD值进行比较。
根管治疗磨牙近中和远中区域的FD值显著高于对照牙(p < 0.05)。治疗时间、牙位以及患者的性别和年龄对FD值增加无统计学显著影响(p > 0.05)。近中FD值变化与治疗年份之间存在相关性(r = - 0.21,p = 0.035)。
根管治疗1年后,在下颌后牙所有年龄组和性别的牙齿近中和远中区域的小梁骨中均观察到可定量测量的结构变化。
