Twin-reversed arterial perfusion (TRAP) sequence is a rare complication of monochorionic twin gestations characterised by an acardiac twin receiving retrograde perfusion from a structurally normal pump twin via abnormal vascular anastomoses. We report the case of a 20s gravida 2, para 1 woman who underwent ultrasound-guided percutaneous microwave ablation (MWA) at 25 weeks of gestation to interrupt flow to the intra-abdominal umbilical cord vessels of the acardiac twin. Successful cessation of blood flow was achieved. This case highlights the feasibility and clinical potential of MWA as an alternative to conventional modalities in the management of TRAP sequence.
双胎反向动脉灌注(TRAP)序列是单绒毛膜双胎妊娠的一种罕见并发症,其特征是无心双胎通过异常血管吻合从结构正常的泵血双胎接受逆行灌注。我们报告了一例20多岁、孕2产1的妇女,她在妊娠25周时接受了超声引导下经皮微波消融(MWA),以中断流向无心双胎腹腔内脐血管的血流。成功实现了血流停止。该病例突出了MWA作为TRAP序列管理中传统方法替代方案的可行性和临床潜力。
