We treated six children with neuroblastomas. They were all refractory to conventional chemotherapy or at an advanced stage when diagnosed. Patients were four males and two females aged 1 to 7 years old, with a mean age of 3.7 years. Total cis-DDP dose ranged from 60 mg/m2 to 720 mg/m2, with a median dose of 240 mg/m2, and a mean dose of 260 mg/m2. Of the six children, two were PR, three were PD, and one was NC. Of three non-responses, two cases were clinically improved. Two cases had renal dysfunction but this improved within 3 weeks. It is said that cis-DDP induces irreversible renal damage, especially tubular necrosis. Our two cases had progressively decreased creatinine clearance, though values of BUN and creatinine were normal. Bone marrow suppression was mild, so there was no need to discontinue or modify the cis-DDP chemotherapy schedules. Two hypo-magnesemia were found, both cases being asymptomatic. Transient liver dysfunction occurred in one child, and nausea and vomiting were observed in almost all cases.
我们治疗了6例神经母细胞瘤患儿。他们在诊断时均对传统化疗耐药或处于晚期。患儿年龄1至7岁，4例男性，2例女性，平均年龄3.7岁。顺铂总剂量为60mg/m²至720mg/m²，中位剂量为240mg/m²，平均剂量为260mg/m²。6例患儿中，2例部分缓解（PR），3例疾病进展（PD），1例疾病稳定（NC）。在3例无反应的患儿中，2例临床症状改善。2例出现肾功能不全，但在3周内恢复。据说顺铂可导致不可逆的肾损伤，尤其是肾小管坏死。我们的2例患儿肌酐清除率逐渐下降，不过尿素氮和肌酐值正常。骨髓抑制较轻，因此无需中断或调整顺铂化疗方案。发现2例低镁血症，均无症状。1例患儿出现短暂肝功能障碍，几乎所有病例均观察到恶心和呕吐。
