The significance of addressing labral tears cannot be overstated given that the labrum plays a crucial role in preventing or mitigating the onset of osteoarthritis. Traditionally, labral repair has involved capsule elevation followed by acetabular rim trimming and decortication to optimize healing of the labrum to the acetabular rim. However, in certain patients, such as those with borderline dysplasia, the goal is to preserve as much of the acetabular rim as possible to prevent further instability. This study aims to present an arthroscopic marrow venting technique designed to optimize healing during hip labral repair. The procedure involves targeted marrow stimulation by micro-drilling along the acetabular rim to release and activate biological healing factors, thereby promoting the integration of the labrum and restoring the seal around the hip. This approach eliminates the need for capsule elevation and rim trimming, which preserves bone and capsular tissue and reduces the risk of iatrogenic instability.
