Vital tooth preparation for dental prosthesis can lead to apical periodontitis due to factors such as exposed dentinal tubules, poor prosthesis fit, and inadequate oral hygiene. These issues often necessitate root canal treatment. However, removing prosthesis is challenging, particularly when patients hesitate due to cost, complicating treatment decisions. This case series explores the use of the AReneto® system, an augmented reality (AR)-guided approach, for minimally invasive access through existing prosthesis.
Two patients with porcelain-fused-to-metal (PFM) bridge prosthesis presented with pain and periapical pathology. Preoperative virtual planning of the access cavity was performed. The planned access was projected into the clinician's field of view via AReneto® system with the help of the AR glasses to provide guidance during the treatment procedure.
The AReneto® system enabled accurate access through the existing prosthesis. The root canals were cleaned, shaped, and obturated successfully. Follow-up appointments showed resolution of symptoms and healing of periapical pathology, confirming the effectiveness of this minimally invasive approach.
