Pseudoaneurysm of the mitral-aortic intervalvular fibrosa (MAIVF) may develop as a complication of endocarditis or aortic valve surgery. Aortic regurgitation (AR) may contribute to the development of a pseudoaneurysm as the jet weakens the MAIVF region. We present a case of MAIVF aneurysm with an unusual mechanism of AR without any valvular or root pathology, delineated clearly by transesophageal echocardiography (TEE) intraoperatively warranting a change in surgical plan to spare the valve instead of the pre-planned aortic valve replacement.
