Abstract

BACKGROUND: Chemotherapy-induced peripheral neuropathy (CIPN) is a progressive and persistent condition that can significantly affect the quality of life of patients. Even after the cessation of chemotherapy, symptoms of CIPN may endure for an extended period. Despite its considerable impact, there is a paucity of research examining the effects of CIPN on patients. OBJECTIVE: To investigate the current status of CIPN in patients with nasopharyngeal carcinoma (NPC) and accurately identify factors influencing neurotoxicity based on the Random Forest algorithm. METHOD: A total of 289 patients with NPC were admitted to the Radiotherapy Department of the First Affiliated Hospital of Guangxi Medical University between August 2023 and April 2024 to investigate the current status of neurotoxicity, cognitive impairment, fatigue, and hand-foot syndrome. To rank the importance of the influencing factors of CIPN in NPC patients, a random forest model was constructed to analyze the influencing factors. RESULTS: The incidence of CIPN in NPC patients was 81.7%. The results of the random forest algorithm showed that age, chemotherapy cycle, tumor stage, chemotherapy regimen, concurrent chemoradiotherapy, BMI, hand and foot syndrome fatigue, and cognitive dysfunction were the main factors affecting CIPN in NPC patients (P<0.05). CONCLUSION: There is an urgent need to accurately identify the risk factors for CIPN in patients with NPC and develop multidisciplinary collaborative prevention and intervention measures to achieve effective prevention and comprehensive management.

背景:化疗引起的周围神经病变(CIPN)是一种进行性且持续的病症,会显著影响患者的生活质量。即使化疗停止后,CIPN的症状仍可能持续较长时间。尽管其影响重大,但针对CIPN对患者影响的研究却很匮乏。 目的:调查鼻咽癌(NPC)患者中CIPN的现状,并基于随机森林算法准确识别影响神经毒性的因素。 方法:2023年8月至2024年4月期间,共有289例NPC患者入住广西医科大学第一附属医院放疗科,以调查神经毒性、认知障碍、疲劳和手足综合征的现状。为了对NPC患者中CIPN影响因素的重要性进行排序,构建了随机森林模型来分析影响因素。 结果:NPC患者中CIPN的发生率为81.7%。随机森林算法结果显示,年龄、化疗周期、肿瘤分期、化疗方案、同步放化疗、BMI、手足综合征、疲劳和认知功能障碍是影响NPC患者CIPN的主要因素(P<0.05)。 结论:迫切需要准确识别NPC患者中CIPN的危险因素,并制定多学科协作的预防和干预措施,以实现有效的预防和综合管理。
