Abstract

OBJECTIVE

To explore the barriers faced by early-career nurses in practicing narrative nursing, covering individual competence, patient cooperation, institutional support, and sociocultural factors, aiming to provide theoretical and practical references for enhancing nurses' humanistic care ability, optimizing nurse-patient relationships, and improving care quality.

METHODS

A phenomenological qualitative study was conducted using purposive sampling to select seven early-career nurses from the Second Affiliated Hospital of Xi'an Jiaotong University between March and June 2024. Semi-structured face-to-face interviews were performed, and data were analyzed using Colaizzi's seven-step method to extract themes.

RESULTS

Four major themes and ten subthemes were identified: (1) weak narrative competence among early-career nurses, manifested as the gap between theory and practice, insufficient communication in complex situations, and difficulty in emotional regulation; (2) limited patient understanding and cooperation, including communication barriers and lack of trust; (3) constrained objective conditions, such as high workload, lack of training, and insufficient humanistic atmosphere; (4) sociocultural factors affecting narrative nursing implementation, including imbalance between treatment and humanistic needs and reduced nurse-patient interaction willingness due to negative events.

CONCLUSION

Insufficient narrative competence of early-career nurses and low patient cooperation are the main obstacles to promoting narrative nursing. Targeted training, improving patient awareness, strengthening institutional support, and fostering a positive humanistic environment can effectively enhance nurses' humanistic care abilities and optimize nurse-patient relationships.

目的

探讨初职护士在开展叙事护理时面临的障碍,涵盖个人能力、患者配合、机构支持和社会文化因素,旨在为提高护士人文关怀能力、优化护患关系及提升护理质量提供理论和实践参考。

方法

采用现象学质性研究方法,通过目的抽样,于2024年3月至6月从西安交通大学第二附属医院选取7名初职护士。进行半结构化面对面访谈,采用Colaizzi七步法对数据进行分析以提炼主题。

结果

识别出四个主要主题和十个子主题:(1)初职护士叙事能力薄弱,表现为理论与实践脱节、复杂情境下沟通不足以及情绪调节困难;(2)患者理解与配合有限,包括沟通障碍和缺乏信任;(3)客观条件受限,如工作量大、缺乏培训以及人文氛围不足;(4)影响叙事护理实施的社会文化因素,包括治疗与人文需求失衡以及负面事件导致护患互动意愿降低。

结论

初职护士叙事能力不足和患者配合度低是推进叙事护理的主要障碍。针对性培训、提高患者认知、加强机构支持以及营造积极的人文环境可有效提高护士人文关怀能力并优化护患关系。
