Abstract

We report the cases of two extremely preterm infants who were successfully treated with tissue plasminogen activator (tPA) for a life-threatening obstructive intraluminal hematoma. One infant presented with bowel perforation and ileus resulting from a gastrointestinal (GI) hematoma, whereas the other experienced lung collapse due to an intratracheal hematoma. Direct administration of tPA to the GI tract and trachea, respectively, resulted in hematoma lysis and relief of the obstruction. For extremely preterm infants with limited viable treatment options, local tPA may be a valuable approach for managing severe obstructive intraluminal hematomas that lead to GI obstruction or lung collapse.

我们报告了两例极早产儿的病例,他们因危及生命的管腔内阻塞性血肿而成功接受了组织型纤溶酶原激活剂(tPA)治疗。一名婴儿因胃肠道(GI)血肿出现肠穿孔和肠梗阻,而另一名婴儿因气管内血肿出现肺萎陷。分别将tPA直接注入胃肠道和气管,导致血肿溶解和梗阻缓解。对于可行治疗选择有限的极早产儿,局部使用tPA可能是治疗导致胃肠道梗阻或肺萎陷的严重管腔内阻塞性血肿的一种有价值的方法。
