We aimed to investigate the correlations of platelet distribution width (PDW) and serum iron metabolism indicators with cardiac function and inflammatory response in patients with chronic heart failure (CHF) and their predictive values for poor prognosis.
Eighty-four patients with CHF treated from August 2022 through September 2024 were retrospectively selected as the HF group, while eighty-eight healthy people with normal cardiac function during the same period were selected as the control group. PDW, serum ferritin (SF), transferrin saturation (TS), cardiac function indicators, and inflammatory response were compared between the two groups.
The levels of interleukin-6 (IL-6), C-reactive protein (hs-CRP), and tumor necrosis factor-α (TNF-α) in the HF group were significantly higher than in the control group (p < 0.05). The results of the Pearson correlation analysis revealed that left ventricular ejection fraction (LVEF) was negatively correlated with PDW and positively correlated with SF and TS. The IL-6, hs-CRP, and TNF-α levels all had a positive correlation with PDW and negative correlations with SF and TS (p < 0.05). The areas under the receiver operating characteristic (ROC) curve of PDW, SF, TS, and combination of the three for predicting the prognosis of CHF patients were 0.784, 0.781, 0.715, and 0.874, respectively, suggesting that combined detection possesses certain clinical value.
The combination of PDW, SF, and TS has good clinical value for predicting the prognosis of patients with CHF, which can assist in the evaluation of cardiac function and prognosis of CHF patients.

回顾性选取2022年8月至2024年9月期间接受治疗的84例CHF患者作为心力衰竭组,同时选取同期88例心功能正常的健康人作为对照组。比较两组的PDW、血清铁蛋白(SF)、转铁蛋白饱和度(TS)、心功能指标及炎症反应。
心力衰竭组白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)和肿瘤坏死因子-α(TNF-α)水平显著高于对照组(p<0.05)。Pearson相关分析结果显示,左心室射血分数(LVEF)与PDW呈负相关,与SF和TS呈正相关。IL-6、hs-CRP和TNF-α水平均与PDW呈正相关,与SF和TS呈负相关(p<0.05)。PDW、SF、TS及三者联合预测CHF患者预后的受试者工作特征(ROC)曲线下面积分别为0.784、0.781、0.715和0.874,提示联合检测具有一定临床价值。
PDW、SF和TS联合检测对预测CHF患者预后具有良好的临床价值,可辅助评估CHF患者的心功能及预后。
