Schwannomas of the hand are uncommon, and those arising from the thenar motor branch are extremely rare. Their indolent nature and overlap with ganglion cysts often delay accurate diagnosis.
A 40-year-old woman presented with a gradually progressive, painless thenar swelling, initially presumed to be a ganglion. Ultrasonography and magnetic resonance imaging failed to yield definitive findings. The mass was enucleated carefully. Histopathological examination confirmed schwannoma. The patient made a complete functional recovery.
Schwannomas in the thenar region may mimic ganglia. Imaging may not always be diagnostic. When symptoms suggest neural involvement, surgical exploration and biopsy are warranted to avoid misdiagnosis and preserve function.
手部神经鞘瘤并不常见,起源于鱼际肌运动分支的神经鞘瘤极为罕见。其生长缓慢的特性以及与腱鞘囊肿的症状重叠常常导致准确诊断的延迟。
一名40岁女性因鱼际肌逐渐进行性、无痛性肿胀就诊,最初被诊断为腱鞘囊肿。超声和磁共振成像未能得出明确结果。该肿物被仔细摘除。组织病理学检查确诊为神经鞘瘤。患者功能完全恢复。
鱼际区域的神经鞘瘤可能类似腱鞘囊肿。影像学检查不一定总能确诊。当症状提示神经受累时,进行手术探查和活检以避免误诊并保留功能是必要的。
