Abstract

INTRODUCTION

Sepsis-associated acute kidney injury (SA-AKI) is a frequent complication in critically ill patients and is associated with increased mortality. This study aimed to evaluate the prognostic value of the lactate/albumin ratio (LAR) and other albumin-based biomarkers in predicting SA-AKI.

METHODS

A retrospective observational study was conducted on 564 patients with sepsis, divided into two groups based on the presence (n = 298) or absence (n = 266) of AKI. Clinical characteristics, laboratory parameters, and disease severity scores (SOFA, APACHE II, NEWS, NEWS-Lactate) were compared between groups. The predictive performance of each marker in detecting SA-AKI was assessed using receiver operating characteristic (ROC) curve analysis.

引言

脓毒症相关急性肾损伤(SA - AKI)是重症患者常见的并发症,且与死亡率增加相关。本研究旨在评估乳酸/白蛋白比值(LAR)及其他基于白蛋白的生物标志物在预测SA - AKI方面的预后价值。

方法

对564例脓毒症患者进行回顾性观察研究,根据是否存在急性肾损伤(AKI)分为两组(存在AKI组,n = 298;不存在AKI组,n = 266)。比较两组间的临床特征、实验室参数及疾病严重程度评分(序贯器官衰竭评估(SOFA)、急性生理与慢性健康状况评分系统II(APACHE II)、国家早期预警评分(NEWS)、含乳酸的国家早期预警评分(NEWS - Lactate))。使用受试者工作特征(ROC)曲线分析评估每个标志物检测SA - AKI的预测性能。
