Bilateral medial medullary infarction (BMMI) is a rare type of brainstem ischemic stroke involving the anteromedial medulla oblongata bilaterally. Diagnosis is often delayed due to its rarity, non-specific clinical presentation, and non-revealing imaging in the early phases. MRI plays a crucial role in early detection by revealing characteristic patterns. We report two cases of BMMI, highlighting classic and additional magnetic resonance imaging (MRI) findings and their anatomic correlations.
