Adrenal capillary hemangiomas are an extremely rare tumor type, and no previous studies have described their features using dynamic contrast-enhanced computed tomography. We report the case of a 65-year-old male patient with a history of right partial nephrectomy for renal clear cell carcinoma. During follow-up, computed tomography scans revealed growth of a right adrenal nodule. On contrast-enhanced computed tomography, the adrenal tumor showed peripheral nodular and progressive central enhancements in the early and delayed phases, respectively. No evidence of metastasis to other organs appeared on computed tomography, and hormone levels showed no elevation in the blood test results. Histopathological examination of the resected adrenal tumor specimen confirmed the presence of clusters of thin-walled vessels, leading to a diagnosis of adrenal capillary hemangioma. This case suggests that adrenal capillary hemangiomas can present imaging findings similar to those previously reported for cavernous adrenal, other adrenal, and capillary hemangiomas originating from other organs.
