Objective To investigate the expression levels of class Ⅰ histone deacetylases(HDAC)in the serum of patients with newly diagnosed psoriatic arthritis(PsA)and screen out serological indicators that are of significance for early diagnosis and assessment of disease activity.Methods A total of 49 PsA patients newly diagnosed in Shanxi Provincial People's Hospital from August 2022 to February 2024 and 30 healthy individuals(control group)were enrolled in this study.Demographic data were collected,and disease severity was assessed.Serum samples were collected,and the expression levels of class Ⅰ HDAC(HDAC1,HDAC2,HDAC3,and HDAC8)in the serum of each group were determined by ELISA.The correlations between the expression levels of class Ⅰ HDAC and clinical assessment indicators in each group were evaluated.Multivariate Logistic regression was adopted to analyze the risk factors affecting the disease activity of PsA patients.The receiver operating characteristic curve was used to evaluate the diagnostic efficacy of the risk factors affecting the disease activity of PsA patients.Results Compared with the control group,PsA patients showed up-regulated expression levels of HDAC1(=0.003),HDAC2(=0.010),HDAC3(=0.003),and HDAC8(=0.018)in the serum.The serum HDAC1 level of PsA patients was positively correlated with erythrocyte sedimentation rate(=0.344,=0.028).The serum HDAC2 level was positively correlated with the overall assessment of disease activity(=0.468,=0.001),the disease activity index of arthritis(=0.401,=0.007),the number of swollen joints(=0.308,=0.042),hospital anxiety and depression scale(HADS)score of anxiety(=0.360,=0.018),and HADS score of depression(=0.302,=0.047).The serum HDAC3 level was correlated with erythrocyte sedimentation rate(=0.542,<0.001),C-reactive protein(CRP)level(=0.440,<0.001),HADS score of anxiety(=0.420,=0.005),interleukin-6 level(=0.397,=0.004),the overall assessment of disease activity(=0.318,=0.036),and the course of psoriatic arthritis(=0.330,=0.028).The serum HDAC8 level was positively correlated with HADS score of anxiety(=0.477,=0.008)and erythrocyte sedimentation rate(=0.385,=0.039).Compared with the patients with low disease activity,those with moderate to high disease activity presented up-regulated expression of HDAC3(=0.041).HDAC2(=0.028)and CRP(=0.034)were risk factors for moderate to high disease activity in PsA patients.HDAC2(area under the curve=0.802,=0.003)and CRP(area under the curve=0.718,=0.033)had diagnostic value for the progression of PsA.Conclusions The expression levels of class Ⅰ HDAC in the serum of patients with newly diagnosed PsA were significantly different.The serum levels of HDAC2 and CRP are expected to become serological indicators for the early diagnosis and disease activity assessment of PsA.
目的 探讨新诊断银屑病关节炎(PsA)患者血清中Ⅰ类组蛋白去乙酰化酶(HDAC)的表达水平,筛选出对早期诊断及病情活动度评估有意义的血清学指标。方法 选取2022年8月至2024年2月在山西省人民医院新诊断的49例PsA患者及30名健康个体(对照组)。收集人口学资料,评估疾病严重程度。采集血清样本,采用酶联免疫吸附试验(ELISA)检测各组血清中Ⅰ类HDAC(HDAC1、HDAC2、HDAC3和HDAC8)的表达水平。评估Ⅰ类HDAC表达水平与各组临床评估指标的相关性。采用多因素Logistic回归分析影响PsA患者病情活动度的危险因素。绘制受试者工作特征曲线(ROC曲线),评估影响PsA患者病情活动度危险因素的诊断效能。结果 与对照组相比,PsA患者血清中HDAC1(P = 0.003)、HDAC2(P = 0.010)、HDAC3(P = 0.003)和HDAC8(P = 0.018)表达水平上调。PsA患者血清HDAC1水平与红细胞沉降率呈正相关(r = 0.344,P = 0.028)。血清HDAC2水平与疾病活动度总体评估呈正相关(r = 0.468,P = 0.001)、关节炎疾病活动指数呈正相关(r = 0.401,P = 0.007)、肿胀关节数呈正相关(r = 0.308,P = 0.042)、医院焦虑抑郁量表(HADS)焦虑评分呈正相关(r = 0.360,P = 0.018)、HADS抑郁评分呈正相关(r = 0.302,P = 0.047)。血清HDAC3水平与红细胞沉降率(r = 0.542,P < 0.001)、C反应蛋白(CRP)水平(r = 0.440,P < 0.001)、HADS焦虑评分(r = 0.420,P = 0.005)、白细胞介素 - 6水平(r = 0.397,P = 0.004)、疾病活动度总体评估(r = 0.318,P = 0.036)及银屑病关节炎病程呈正相关(r = 0.330,P = 0.028)。血清HDAC8水平与HADS焦虑评分呈正相关(r = 0.477,P = 0.008)和红细胞沉降率呈正相关(r = 0.385,P = 0.039)。与低病情活动度患者相比,中高病情活动度患者HDAC3表达上调(P = 0.041)。HDAC2(P = 0.028)和CRP(P = 0.034)是PsA患者中高病情活动度的危险因素。HDAC2(曲线下面积 = 0.802,P = 0.003)和CRP(曲线下面积 = 0.718,P = 0.033)对PsA病情进展有诊断价值。结论 新诊断PsA患者血清中Ⅰ类HDAC表达水平有显著差异。血清HDAC2和CRP水平有望成为PsA早期诊断及病情活动度评估的血清学指标。
