Renal injuries require immediate or early surgical intervention for threatened exsanguination, damage to the pedicle vessels and disruption of the renal pelvis. Elective delayed operation prevents late complications in those patients in whom there is transparenchymal connection between the pelvocaliceal system and the perirenal space following trauma. There appears to be an increased incidence of nephrectomy if the latter are explored early. Expectant treatment is appropriate for renal lacerations without extravasation, if there is no threat of exsanguination and if the pedicle is intact.
对于有大出血风险、肾蒂血管损伤及肾盂破裂的肾损伤患者,需要立即或尽早进行手术干预。对于外伤后肾盂肾盏系统与肾周间隙存在实质贯通的患者,选择性延迟手术可预防晚期并发症。如果早期探查此类患者,肾切除的发生率似乎会增加。对于无出血风险、肾蒂完整且无外渗的肾裂伤患者,保守治疗是合适的。
