In the severest forms of emphysema the distance between neighbouring alveolar septa inserting to the walls of non respiratory bronchioles increases, and that by their disappearance. This reduces one of the externally acting forces which keep the bronchiole open, which subsequently results in secondary bronchial narrowing.
