In primary closed and simple fractures the local bone healing type--primary or secondary bone healing--after internal fixation depends on the stability/rigidity which has been achieved and secured. This fact is not only of scientific interest but is also important and essential for a correct assessment of the postoperative clinical course. This is demonstrated and discussed by means of intramedullary nailing, which is an unstable osteosynthesis but which allows early loading.
