The involvement of endocervical glands by the Arias-Stella reaction was investigated in 191 hysterectomy specimens obtained during pregnancy. The prevalence of the condition was 9% (17 of 191). The lesion was usually focal in nature and was more commonly present in the more proximal portions of the endocervical canal. The histologic appearance was markedly similar to the one in the endometrium. Nuclear enlargement and atypia, as well as a prominent hobnail pattern, were seen. Nuclear cytoplasmic inclusions and a glassy appearance of involved nuclei were occasionally observed.
