Fourth nerve pareses are the most frequently seen. Treatment is aimed at correcting the deviation according to a classification based on the spread of comitance of the deviation. With the recent addition of the Harada-Ito operation and the revival of the use of adjustable sutures, the choice of operative procedure has been widened to give even better results.
