An acutely tender painful testis is likely to be a torsion, particularly in the boy before or at the time of puberty. In the older patient the lie of the other testis may give a clue to the diagnosis of torsion and at any age the history of previous "warning" episodes should be sought. Operation as soon as possible and always within 5 hours of the onset of pain is mandatory. The other testis should always be fixed. There is no place for treating small boys with antibiotics on the assumption of a diagnosis of epididymitis.
急性触痛的睾丸很可能是扭转,尤其是青春期前或青春期时的男孩。对于年龄较大的患者,对侧睾丸的位置可能有助于扭转的诊断,且在任何年龄都应询问既往“警示”发作史。必须尽快手术,且总是在疼痛发作后5小时内进行。对侧睾丸也应始终固定。绝不能在假定诊断为附睾炎的情况下用抗生素治疗小男孩。
