In contrast to pseudoexfoliation, true exfoliation is an extremely rare condition not associated with glaucoma. In true exfoliation the superficial layers of the anterior lens capsule are partially peeled off. This results in a fine homogenous water-clear transparent membrane floating in the anterior chamber. The etiology of this mostly bilaterally condition may be exposition to infrared rays, iridocyclitis, metallic intraocular foreign bodies and trauma. To our knowledge this is the first published report of a case of idiopathic true exfoliation of the lens capsule.
