This is a report about a 57 year old woman with Peutz-Jeghers syndrome and adenocarcinoma at the Flexura duodenojejunalis. Several possibilities of the origin of carcinoma are discussed. In the present case the origin is due to a Peutz-Jeghers polyp. The risk of malignant change in Peutz-Jeghers syndrome is about 2-3%. If specific clinical symptoms are present one should think of such a rare possibility of complication. Special screening-tests do not seem to be necessary. With mucinhistochemical methods we found a decrease of neutral mucins in Peutz-Jeghers polyps and in a higher degree in dysplastic and carcinomatous areas. A final explanation of this phenomenon cannot be given at present.
