A case with a variant of the subclavian steal syndrome associated with an anomalous origin of the left vertebral artery from the aortic arch is presented. Prior to the examination a carotid-subclavian bypass had been performed. The main angiographic finding is the blood supply of the distal subclavian artery through an ipsilateral cervical collateral system. Angiographic procedure and therapeutic consequences are discussed.
