We reviewed the clinical and renal biopsy information of 115 patients who presented with renal disease after the age of sixty years. The major clinical presentations were renal insufficiency in 57 patients, nephrotic syndrome in 35 patients, and hematuria with variable amounts of proteinuria in 23 patients. Crescentic glomerulonephritis, diagnosed in 19 patients, was the most common cause of renal insufficiency. Membranous glomerulonephritis and minimal change nephrotic syndrome were noted in 15 and 9 patients, respectively. While patients with crescentic glomerulonephritis had an unfavorable outcome, patients with minimal change nephrotic syndrome and diffuse proliferative glomerulonephritis had a benign course. We conclude that even in the elderly, the natural history of renal disease is similar to that of younger patients, and the clinical course appears to depend on the nature of the underlying renal disease.
我们回顾了115例60岁以后出现肾脏疾病患者的临床及肾活检信息。主要临床表现为57例肾功能不全、35例肾病综合征、23例伴有不同程度蛋白尿的血尿。19例诊断为新月体性肾小球肾炎,是肾功能不全最常见的病因。分别有15例和9例患者被诊断为膜性肾小球肾炎和微小病变肾病综合征。新月体性肾小球肾炎患者预后不佳,而微小病变肾病综合征和弥漫性增生性肾小球肾炎患者病程呈良性。我们得出结论,即使在老年人中,肾脏疾病的自然病程与年轻患者相似,临床病程似乎取决于潜在肾脏疾病的性质。
