We investigated two cases of recurrent jaundice, both involving repeated exposure to phenprocoumon and suggestive of coumarin-induced hepatitis. The first patient showed the histologic picture of cholestasis during the second bout of jaundice, the other that of chronic aggressive hepatitis. In both patients, the hepatitis appearance disappeared after termination of the first exposure to the coumarin drug, in the first case without additional drug therapy. In the second case, prednisone therapy was instituted when phenprocoumon administration was withdrawn. In both cases, hepatopathy recurred after reexposure to the drug. In one of these patients, and with his consent, a third bout of jaundice was provoked with phenprocoumon.
我们研究了两例复发性黄疸病例,均涉及反复接触苯丙香豆素,提示为香豆素诱导的肝炎。首例患者在第二次黄疸发作时表现出胆汁淤积的组织学特征,另一例则表现为慢性侵袭性肝炎。在这两名患者中,首次接触香豆素药物终止后,肝炎表现均消失,首例患者未接受额外药物治疗。在第二例患者中,停用苯丙香豆素后开始使用泼尼松治疗。两例患者再次接触该药物后肝病均复发。在其中一名患者同意的情况下,用苯丙香豆素诱发了第三次黄疸发作。
