The authors in this work report a further case of idiopathic dilatation of the colon which was complicated by perforation of the caecum following a Caesarean operation. The aetiology of this condition is uncertain, the diagnosis difficult and the prognosis poor. It is necessary to try to operate before the caecum perforates and to do this by decompression, which should for preference be carried out at a site in the transverse colon.
