We report a case of laparoscopic radical hysterectomy with laparoscopic paraaortic and pelvic lymphadenectomy for treatment of cervical cancer FIGO stage IAI in a 26 year old patient with adipositas permagna (Quetelet-Index 40.6). Surgery took 5 hours and 55 minutes with an estimated blood loss of 1000 cc, 27 lymph nodes were resected and the patient was discharged on postoperative day 5 having completely recovered. Investment in further development of this technique in comparison with established methods seems justified.
