Experience in surgical treatment of more than 16 thousand patients with acute paraproctitis is discussed. In most cases the distal part of the main purulent passage comes out of the cavity of the abscess into the rectal lumen inside the sphincter. This makes possible in most cases simultaneous elimination of the inner opening of the paraproctitis in the anal canal.
