The presentation of an adrenal cortical carcinoma with myxoid ground substance is an unusual and apparently rare event. To my knowledge, only one such case has been previously reported in the literature. A second case of a metastasizing adrenal cortical carcinoma with abundant acidic mucosubstances and a myxomalike appearance is reported herein. The myxoid variant of adrenal cortical carcinoma should be included in the differential diagnosis of retroperitoneal myxoid tumors.
