Microsurgery of cryptorchidism. I: Lengthening the short vas deferens.

About 70% of patients with cryptorchidism have a vas deferens that is so short that ideal scrotal positioning of the testis cannot be achieved, even with microvascular lengthening of the testicular vascular pedicle. A new method of microvascular testicular autotransplantation is presented for those patients with short vasa. The technique involves the complete dissection of the vas deferens from the retrovesical space down to the epididymodeferential loop and transposition of the testis into the scrotum in the upside-down position. By this maneuver, extra length is gained equal to the longitudinal dimension of the testis.
