The occurrence of microanastomotic thrombosis in autotransplantation of the cryptorchid testis seems to result from low runoff in the small vascular bed of the transplanted testis. A new method for testicular revascularization is presented to compensate for the customary three- to fivefold caliber discrepancy between the testicular and deep inferior epigastric arteries. An arteriovenous fistula is created between the deep inferior epigastric artery and a vena comitans to reduce outflow resistance and normalize flow rates between the feeding artery and its runoff bed.
