Comprehensive healthcare reform depends on our ability to educate more primary care providers and reduce the oversupply of specialists and subspecialists. However, significant financial incentives exist, hidden within the Federal reimbursement system to fuel continued growth of highly specialized physicians, undermining efforts to educate primary care practitioners. If Medicare cannot correct this situation, perhaps the government should redirect GME funds to educate nurse practitioners.
