Physiological disturbances can be caused by loss of large intestine, ileocecal valve and half of anal canal mucosa. Pouch can not compensate the whole colonic absorptive function and patients have to live with some of functional abnormality. Pouch should be made as large as possible with good compliance to maintain reservoir function. Post-operative anal complications also should be minimized to prevent anal dysfunction caused by late fistula or stricture.
