免疫放射分析法测定促甲状腺激素及促甲状腺激素释放激素试验的临床意义

Abstract

The levels of serum TSH were measured by a sensitive immunoradiometric assay (IRMA) method and the responses of TSH to TRH stimulation were observed in three groups of hyperthyroidism, primary hypothyroidism and secondary hypothyroidism. The levels of serum TSH were found to be undetected in 98% (1/51) of the patients with hyperthyroidism, very high in 100% (35/35) of the patients with primary hypothyroidism and normal in 91% (30/33) of the patients with secondary hypothyroidism. TRH test showed no responses of TSH in all patients with hyperthyroidism, high responses in all patients with primary hypothyroidism and blunt responses in 69% (11/16) of patients with secondary hypothyroidism. The results indicate that the measurement of serum TSH by IRMA is a sensitive index in the diagnosis of hyperthyroidism and primary hypothyroidism, but can not be differentiated the secondary hypothyroidism from normal conditions. TRH test may be helpful in the differential diagnosis between secondary hypothyroidism and normal conditions.

采用灵敏的免疫放射分析(IRMA)法测定三组甲亢、原发性甲减和继发性甲减患者的血清促甲状腺激素(TSH)水平,并观察TSH对促甲状腺激素释放激素(TRH)刺激的反应。结果发现,98%(51例中的1例)甲亢患者血清TSH水平未检测到,100%(35例中的35例)原发性甲减患者血清TSH水平非常高,91%(33例中的30例)继发性甲减患者血清TSH水平正常。TRH试验显示,所有甲亢患者的TSH均无反应,所有原发性甲减患者的TSH反应高,69%(16例中的11例)继发性甲减患者的TSH反应迟钝。结果表明,用IRMA法测定血清TSH是诊断甲亢和原发性甲减的灵敏指标,但不能区分继发性甲减与正常情况。TRH试验可能有助于继发性甲减与正常情况的鉴别诊断。
