A comprehensive overview is presented on cerebral hemodynamics and metabolism in intensive care, with emphasis on cerebral oxygenation monitoring and management. From blood pressure to the most recently introduced concept of cerebral hemometabolism, cerebral hemodynamic reserve, concepts, variables, and findings are addressed. Integrated cerebral hemodynamics and metabolism, that is, cerebral hemometabolism, is discussed beyond cerebral perfusion pressure and blood flow. Monitoring and management of cerebral oxygenation and related variables are emphasized, in terms of current and potential future applications.
