Based on an atypical case of squamous carcinoma of the tongue in a young non-alcoholic, non-smoking woman, the authors review the anatomy of the tongue and adjacent spaces. The features of extension and the CT and MRI diagnosis of the tumour are analysed. Extension beyond the oropharynx to the pharyngolarynx and intratemporal fossa and bony involvement changed the stage and modified the treatment.
