Coronary heart disease can be a silent disease and does not always present with symptoms directly attributable to the heart. In this article, a presenting complaint of balanitis led to the diagnosis of diabetes mellitus, which led to the detection of hypertension and hypercholesterolaemia and eventually to the diagnosis of severe coronary heart disease.
