Vigilance for signs of decreasing renal function--especially rising creatinine and dwindling urine output--will permit prompt preventive action. When renal shutdown does occur, the physician first must determine whether it is prerenal, intrarenal, or postrenal in origin and then institute specific restorative interventions to minimize the degree of failure and associated morbidity.
