It is hoped this article has demonstrated that a nursing theoretical model, in conjunction with the MDS and Resident Assessment Protocol Summary (RAPS), may be used effectively to plan care for residents in long-term care facilities. The MDS RAPS format has been included to facilitate this goal (Figure 2). Each of the RAPS problem areas has been broken down into primary, secondary, and tertiary preventions. Suggestions for approaches to the problems through a nursing care plan are listed (see "Levels of Prevention"). It is not an exhaustive list but nurses can build on this base so that resident-nursing goals may be met using concepts easily understood by residents, family, and ancillary help.
