The German health care system is characterized by three trends that will continue into the next millennium: lasting restriction of financial resources, extension of remuneration systems with rationalization incentives; shift of responsibility from the state towards self-administration. These are illustrated by examples. The fundamental positions of the relevant political parties are basically similar. Medical quality assurance measures will grow more important. The adaptation process will only be possible on a step-by-step basis. Physicians have a key role. The major medical organizations are willing to cooperate constructively. This is proof of a great sense of responsibility, but also of the realization that shaping the future is better than standing aloof.
